How to choose a medical answering service

Start with who hears the caller, whether you need a BAA, and what happens after hours. Price comes after those three.

A medical answering service is a person, a script, or a bot that hears your callers when your front desk cannot. The useful comparison is not brand names. It is who hears protected health information (PHI), what they are allowed to say, and what happens at 2 a.m. on a holiday.

Start with the BAA

If the desk will create, receive, maintain, or transmit PHI for your practice, you likely need a business associate agreement before you share that information. The U.S. Department of Health and Human Services (HHS) publishes this in its Business Associates guidance, last reviewed July 30, 2026 on the HHS site. HHS does not run a certification program for answering services.

A vendor homepage that says it is HIPAA-compliant is a marketing sentence. A signed BAA is a contract with required terms at 45 CFR 164.504(e). Live, virtual, AI, or hybrid

Sales pages collapse four desks into one phrase. They are not the same product. The longer briefing is Live operators, virtual receptionists, and AI intake.

  • Live operators answer, follow your script, and escalate. Still the default for messy after-hours medical, hospice, and many veterinary emergency lines.
  • Virtual receptionists are usually daytime overflow: booking, messages, transfers. Ask whether the same person is trained for chest-pain or death calls.
  • AI intake captures a reason for call and sometimes writes back to the EHR. HHS lists a third-party AI chatbot that uses PHI for symptom assessment or scheduling as a business associate.
  • Hybrid can mean a person confirms an AI draft, or it can mean “bot until the caller yells.” Ask for the escalation tree in writing.

After-hours is a different job

Daytime overflow and overnight on-call are easy to buy as one package and hard to staff as one skill. After-hours work is fewer calls, higher stakes, and more PHI in a single message: symptoms, medications, who is on call. If your clinic keeps a physician or nurse on call, read the after-hours specialty page and ask what happens when the script fails: transfer, voicemail, or a dead line.

What to ask before you share PHI

  1. Will you sign a BAA before the first live call, and can counsel see the form first?
  2. Who employs the agent, where do they sit, and which subcontractors hear the same call?
  3. Are calls recorded, how long do recordings and transcripts stay, and who can replay them?
  4. Who is allowed to give any clinical advice, and under what written protocol?
  5. What is the after-hours escalation tree, including holidays and a failed script?
  6. Can you hear a redacted sample of a real after-hours call, not a demo?
  7. What does the invoice include: minutes, messages, wrap-up, patch-through, SMS, EHR write-back?
